There are at least two versions of the Form FA-25/26. One version is only three pages and is identified in the lower left hand corner as "Form FA-25/26." Another version that is four pages identifies itself as "F 25/26 01/04/02" in the lower left hand corner. Question No. 13 in these two versions is different. The "Form FA-25/26" asks: "Has any License to Carry Firearms, Permit to Possess Firearms, or Firearms Identification Card issued under the laws of any state or territory ever been suspended, revoked or denied?" The "F 25/26 01/04/02" asks: "Has any License to Carry Firearms, Permit to Possess Firearms, or Firearms Identification Card issued to you under the laws of any state, territory or jurisdiction ever been suspended, revoked or denied?" (emphasis in bold added). If a person has applied previously for a new LTC and been denied, then applies again, the answer to Question 13 in the "F 25/26 01/04/02" is "No" but the answer to Question 13 in the "Form FA-25/26" would be "Yes." So, does anyone know which form is the latest version or has anyone encountered this issue before?
